Kohl’s Employees Access Mobile IV Therapy
Two Centivo Plans with IVTherapy2go™ Across Texas
When you are not feeling well, traveling to receive supportive medical care may add more stress to your day. IVTherapy2go™ brings medically appropriate IV therapy to eligible patients at home, work, or another approved Texas location.
Kohl’s employees may be enrolled in the High Performance Plan Basic or High Performance Plan Plus. Both plans are administered by Centivo, but their copays, deductible requirements, network rules, and referral requirements differ.
Coverage for mobile IV therapy may depend on medical necessity, the treatment ordered, provider-network participation, prior authorization, and the plan’s current requirements. Verify your benefits before requesting an appointment.
What Kohl’s Medical Plans Are Available?
Kohl’s employees may be enrolled in one of two medical plans:
-
High Performance Plan Basic
-
High Performance Plan Plus
The available plan information lists the following member costs.
High Performance Plan Basic
-
$0 copay for visits to a Kohl’s Wellness Center
-
$0 copay for Centivo virtual primary care
-
$25 copay plus the applicable deductible for primary care
-
$90 copay plus the applicable deductible for specialist care
-
$25 copay plus the applicable deductible for OB/GYN care
High Performance Plan Plus
-
$0 copay for visits to a Kohl’s Wellness Center
-
$0 copay for Centivo virtual primary care
-
$20 copay for primary care
-
$50 copay for specialist care
-
$20 copay for OB/GYN care
These listed benefits do not automatically apply to mobile IV therapy.
IV fluids, medications, vitamins, laboratory testing, medical supplies, mobile services, and the medical evaluation may be processed separately. Members enrolled in the Basic Plan may also need to satisfy the applicable deductible.
Some elective or wellness-focused IV treatments may not be covered by health insurance. A service may require medical necessity, supporting documentation, a provider’s order, or prior authorization to qualify for coverage.
An insurance card does not guarantee coverage or payment. Current eligibility, medical necessity, provider-network participation, referral requirements, deductible status, and the treatment provided may affect your final responsibility.
For scheduling assistance, call IVTherapy2go at (817) 508-8169.
Can Kohl’s Employees Use IVTherapy2go?
Eligible Kohl’s employees and their covered family members may be able to use IVTherapy2go when mobile IV treatment is considered safe and medically appropriate.
After reviewing the patient’s symptoms and medical history, the medical team will determine whether IV treatment can be provided. Depending on the clinical evaluation, supportive care may be considered for concerns such as:
-
Dehydration
-
Nausea or vomiting
-
Reduced fluid intake
-
Certain migraine symptoms
-
Fluid loss related to a non-emergency illness
-
Provider-recommended hydration
-
Certain vitamin or nutrient treatments
-
Recovery support when clinically appropriate
Not every patient or medical concern is appropriate for mobile IV therapy. The medical professional may recommend laboratory testing, an in-person examination, urgent care, or emergency evaluation instead.
Coverage depends on the member’s plan, current eligibility, medical needs, treatment ordered, provider-network participation, service location, and plan requirements.
Call 911 or visit the nearest emergency room for chest pain, severe breathing difficulty, signs of a stroke, fainting, confusion, uncontrolled bleeding, major trauma, severe dehydration with altered awareness, or another life-threatening emergency.

How Do I Verify My IV Therapy Benefits?
Before requesting an appointment, contact Centivo at (855) 785-9389 and ask:
-
Does IVTherapy2go participate with my specific Kohl’s plan?
-
Is mobile IV therapy covered?
-
Must the treatment be medically necessary?
-
What copay, deductible, or coinsurance applies?
-
Does the Basic Plan deductible apply?
-
Are IV fluids, medications, vitamins, and medical supplies covered?
-
Are mobile or convenience fees excluded?
-
Is a provider’s order required?
-
Is prior authorization required?
-
What supporting medical records are needed?
-
Will I have a time-of-service responsibility?
-
Are there service-area or network restrictions?
Explain the medical reason for the requested treatment and provide the name of the IV service when asking about coverage.
Write down the representative’s name, the date of the call, and the reference number provided. Final coverage is determined when Centivo processes the claim.
IVTherapy2go can collect your insurance information and help prepare for the appointment, but Centivo provides the official explanation of plan benefits.
Is Prior Authorization Required?
Prior authorization depends on the treatment, medication, and medical reason for the requested IV service.
Members and providers should contact Centivo at (855) 785-9389 before receiving treatment that may require approval. Providers can also review authorization requirements through the Centivo Provider Portal.
Ask whether authorization must be obtained by the member, ordering provider, or treating provider.
Prior authorization does not guarantee claim payment. Current eligibility, medical necessity, provider-network participation, referral requirements, plan exclusions, supporting documentation, and other conditions may affect how the claim is processed.
If the requested treatment is not covered, ask IVTherapy2go about the available self-pay cost before the appointment.
What Network Does the Plan Use?
Kohl’s High Performance plans use a Centivo-managed provider network that includes participating Baylor Scott & White Quality Alliance providers.
Not every healthcare provider participates with every Centivo plan. Members should confirm that IVTherapy2go and the treating medical professional participate with their specific Kohl’s plan before receiving care.

Visit the Kohl’s Centivo Provider Locator
-

Provide the employer name, exact plan name, member identification number, requested IV treatment, medical reason for the service, and service location when confirming participation.
Are Specialist Referrals Required?
The Kohl’s High Performance plans require a provider referral before beginning new specialty care.
Referral exceptions include:
-
OB/GYN care
-
Mental health care
-
Chiropractic care
-
Laboratory testing
-
X-rays
-

-

-

Mobile IV therapy is not specifically listed as a referral exception in the available plan information. Contact Centivo to determine whether the treatment requires a referral, provider’s order, or prior authorization.
Without a required referral, the member may be responsible for the full cost of care. Providers can submit referrals through the Centivo Provider Portal.
